I hereby authorize Fix My Playstation (FMP) to repair my PS2. I will not
hold FMP responsible for any items lost or stolen during shipping. I understand
that in order to open and examine the unit, FMP will break the Sony Warranty
Seal thus voiding any remaining manufacturer warranty. I understand that
if the PS2 is not economically feasible to be repaired, I will receive a
refund minus the $10 return shipping fee. I understand that an exam and
return shipping fee of $20 per PS2 will be charged if the unit is not repairable
or found to have no defect. I understand that FMP provides a 90 day warranty
on labor and replaced parts on repaired PS2s, and that my PS2 will be sealed
with a tamper-proof warranty seal. I accept full responsibility for any
and all damage done to my PS2 if it is opened following repair. I understand
while FMP attempts to make same-day repairs, some repairs require extensive
labor requiring additional time or parts that may not be immediately available,
and I will not hold FMP responsible for reasonable extensions of repair
time estimates either expressed or implied. I understand
that NO WARRANTY IS GIVEN IF THE SONY SEAL IS BROKEN UPON ARRIVAL AT FMP.
